COVID-19 REOPENING GUIDELINES + CLIENT INFORMATION
I am so excited to welcome back all of my clients. To continue my part in keeping not only the work environment safe, but my clients and the community safe, I ask that you please review the following reopening guidelines.

If you have the following symptoms, please reschedule your appointment​​:

Cold, cough, runny nose, allergies​, sneezing, fever within the last 72 hours before your appointment, or have been exposed to a known contact with COVID-19. If you have traveled within a 2 week timeframe before your appointment, please reschedule.

Please arrive to your appointment with a mask on.

Please use the restroom to wash your hands before entering the treatment room inside the studio.

Your temperature will be checked at the front desk.

Please come to your appointment alone. No exceptions.

Appointments are spaced out so there is proper sanitizing implemented between each client.

If you are a NEW CLIENT, please fill out the attached CLIENT CONSENT FORM and bring it to your appointment with you. This is to minimize the point of contact with the receptionist as much as possible. If you are scheduled for a lash lift and a brow lift, please fill out both forms. If you are only scheduled for one service, please fill out the form that goes with that service. Please do not forget to bring the form(s) with you to your appointment.
